Resumo

Neste trabalho, é feita uma comparação entre as recomendações para o tratamento do aneurisma da aorta abdominal formuladas em 1992 e aquelas publicadas em 2003, através de diferentes grupos de trabalho e sob a égide da Society for Vascular Surgery e da American Association for Vascular Surgery. O teor das duas recomendações é substancialmente diferente na forma e no conteúdo, em virtude dos enormes progressos e desenvolvimentos técnicos que ocorreram no decurso daquele período de tempo, o maior dos quais foi, sem dúvida, a introdução do tratamento endovascular, que não é ainda considerado como alternativa credível ao tratamento cirúrgico convencional, podendo apenas ser recomendado em circunstâncias particulares, bem definidas. O teor das recomendações do ano de 2003 aparenta ser, para além desse fato, fortemente influenciado pelas grandes transformações que igualmente ocorreram na organização e gestão dos cuidados de saúde e pelas poderosas e influentes forças e interesses que gravitam em torno do ato médico, o que permite concluir, inspirado em H.E. Sigerist, que “(...) as transformações nas concepções médicas estão ligadas ao conjunto de cultura e são condicionadas pelas transformações das idéias da época”.

Abstract

In this article, a comparison is made between the 1992 and the 2003 recommendations for the management of abdominal aortic aneurysm, which were published by different subcommittees of the Society for Vascular Surgery and the American Association for Vascular Surgery. Substantive differences, both in format and content, can be found due to the enormous progress and technical achievements which occurred between 1992 and 2003. The most relevant improvement was the introduction of the endovascular treatment, which is not yet regarded as a valid alternative to the conventional surgical treatment, being indicated only in particular and well defined circumstances. Furthermore, the spirit of the 2003 recommendations seems to be heavily influenced by the large transformations in the organization and management of health care services, as well as the powerful influences and interests around the medical practice. This fact allows us to conclude, following Sigerist, that “... changes in medical concepts are tightly connected to the culture as a whole, and are influenced by the changes of the ideas of each epoch”.
